To ensure that the proper coverage is received, it is necessary for a representative to contact you.  To start the process, please begin by following the below steps:

  1. To request our comprehensive patient information packet, please click here.  A representative will contact you and work with you to gather the necessary information and answer any questions you may have.

  2. After we've captured the necessary information, we will also help you with any paperwork needed to acquire your ACCU-CHEK® Combo system, including a Certification of Medical Necessity, which must be signed by your healthcare provider. Once you've completed these simple steps, the rest is easy.
